Powerheads can be death traps for fish. Some pumps move a lot of water and therefore have a strong suction force. If a fish gets too close, it can get injured easily in the pump. This usually only happens for unhealthy fish, but even healthy ones can fall victim. Certain modifications can be made to the pump to increase the suction area and spread the suction force over that larger area.
